Compensation Range: $25/hr - $28/hr commensurate with experience

Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.

In partnership with the Department of Youth & Community Development (DYCD), Housing Works launched the CRED Program, a new specialized 24-week paid training program for justice-involved and justice-impacted individuals ages 25 – 40. The program provides case management, connection to wraparound health services, industry recognized credential trainings, paid internships and employment placement and retention support.

Reporting to the Program Manager, the Case Manager will be responsible for, but not limited to, conducting screenings and intake assessments, co-developing service plans, connecting individuals to wraparound healthcare services, assisting with benefits and entitlements enrollment, providing program retention support and utilizing the agency and funder’s electronic records systems to document client engagement and program outcomes.

This is an ideal opportunity for a reentry services professional with direct-care experience who is looking to develop skills in the area of care coordination and workforce development, and who is enthusiastic about working alongside an interdisciplinary team of like-minded professionals committed to improving outcomes for individuals and communities impacted by the criminal legal system.

Responsibilities:

Primary:

• Manage a caseload of approximately 25-30 clients.
• Meet with clients assigned to caseload a minimum of bi-weekly.
• Conduct initial screening assessments to determine individuals’ eligibility and commitment to program enrollment.
• Complete intake assessments with newly enrolled clients.
• Co-develop service, or treatment, plans with clients.
• Connect clients to wraparound healthcare services needed (Primary care, substance use treatment, mental health services, etc.)
• Assist clients with obtaining vital documents.
• Assist clients with benefits and entitlements enrollment.
• Utilize motivational interviewing techniques to promote engagement and retention.
• Attend and participate in interdisciplinary case conferences.
• Monitor clients’ progress through the program closely and provide individualized support.
• Document all client services and engagement in the designated case records systems.
• Participate in program outreach and recruitment activities.

Other functions that may be required with this position include:

• Attend and promote engagement in required advocacy activities.
• Participate in Housing Works and DYCD trainings as required.
• Display Housing Works core values in all day-to-day functions.

Requirements:

• Bachelor’s Degree required in social services, psychology, social work or other related field.

The following are qualifications and requirements of the position:

• Minimum 1 year of experience providing direct-client services.
• Experience working with individuals with previous involvement in the criminal legal system.
• Strong written documentation and communication skills
• Good problem-solving skills and ability to work autonomously
• Microsoft Office Suite, particularly Excel, required
• Experience utilizing EMR systems.

The following additional competencies are viewed as important to success in this position:

• Preferably bi-lingual in Spanish.
• Excellent written and oral communication skills.
• Excellent or proficient technical skills to perform tasks related to Microsoft Office, email and case management systems.
• Ability to work independently and collaboratively with the team.
• Ability to frequently travel within the boroughs via public transportation.
