Under general supervision, the HOPWA Housing Case Manager provides comprehensive, client-centered case management services to individuals and families living with HIV/AIDS. The primary focus of this position is to improve health outcomes, housing stability, and income attainment through coordinated supportive services. The Case Manager assists clients in obtaining and maintaining stable housing by conducting assessments, developing individualized service plans, coordinating rental assistance, and linking clients to medical, behavioral health, employment, and community resources. This role includes home visits, crisis intervention, documentation in HMIS and Avatar systems, and ensuring compliance with federal HOPWA program guidelines. The Case Manager serves as an advocate, liaison, and supportive presence while promoting client self-sufficiency and dignity.
Job Responsibilities and Performance Standards:
1.  Client Intake & Assessment
•    Support HOPWA eligibility and comprehensive client intake processes.
•    Assess housing, medical, employment, financial, and psychosocial needs.
•    Collect and maintain required documentation and releases of information.
•    Conduct home visits a minimum of twice per month.
•    Conduct phone calls twice per month (alternating weeks with home visits).
2.  Case Management & Support Services
•    Provide high-quality, trauma-informed case management using harm reduction principles.
•    Develop individualized service plans focused on housing stability and improved health outcomes.
•    Provide crisis intervention, emergency shelter coordination, and supportive counseling as needed.
•    Assist clients by scheduling medical appointments and arranging transportation.
•    Encourage client self-reliance while providing appropriate support.
3.  Housing & Program Coordination
•    Connect clients with rental assistance and supportive housing services.
•    Conduct habitability and visual lead-based paint inspections as required.
•    Review fire safety and lead-based paint education with clients.
•    Coordinate with landlords, healthcare providers, and community agencies.
•    Serve as a liaison for limited English proficient clients during clinic visits.
4.  Documentation & Compliance
•    Maintain accurate, timely, and confidential case records.
•    Document all client contacts, home visits, appointment changes, and case notes.
•    Enter data into HMIS and Avatar systems in compliance with federal, state, and local requirements.
•    Monitor client progress toward service plan goals and adjust plans as needed.
5.  Advocacy & Collaboration
•    Educate clients on program guidelines, rights, and responsibilities.
•    Advocate for client access to healthcare, housing, and supportive services.
•    Work collaboratively with interdisciplinary treatment teams and community partners.
•    Attend staff meetings, training, conferences, and workshops as required.
6.  Training & Professional Development
•    Complete a minimum of:
5 hours annually of HIV/AIDS-specific training
5 hours annually of housing/supportive services training
•    Maintain up-to-date knowledge of HIV/AIDS care, case management, and community resources.
QualificationsEssential Skills & Knowledge
Trauma-informed care and harm reduction approaches
Strong organizational, communication, and documentation skills
Ability to work independently with minimal supervision
Cultural humility and experience working with diverse populations
Knowledge of HIV/AIDS-related issues and housing instability
Minimum Qualifications:
Bachelor’s degree in social services or related field (Relevant experience may substitute for formal education)
Individuals with lived experience are strongly encouraged to apply
Valid state driver’s license and access to a personal vehicle
Ability to maintain strict confidentiality
Preferred Qualifications:
1–3 years of experience in homeless or residential services, Behavioral health or community mental health, and/or Housing case management
Experience with electronic health records and HMIS systems
Demonstrated passion for serving individuals living with HIV/AIDS
Physical Demands: While performing the duties of this job, the employee is regularly required to stand; walk; sit; use hands to finger, handle, or feel; reach with hands and arms; stoop, kneel, crouch, or crawl; talk or hear; and taste or smell.  The employee must frequently lift and/or move up to 25 pounds.  Specific vision abilities required by this job include close vision, depth perception, and ability to adjust focus.
