Compensation & Benefits:
Unison Health is seeking a Case Manager to join our Home-Based Treatment (IHBT) team, providing services to youth and families with complex behavioral health needs. This role is ideal for professionals who value flexible, community-based work, small caseloads, and the opportunity to make a long-term impact through short-term, intensive support.
Key Responsibilities & Role Highlights:
Small caseload – serve a limited number of youth and families at a time
Flexible, community-based hours – appointments take place in the home or community
Team-based, collaborative work environment
Extensive training and professional development opportunities
Support for career growth, including supervision for those pursuing licensure
Participation in 24/7 on-call rotation, with premium call pay included
Time-limited, intensive family support with long-term positive outcomes
Deliver intensive home-based case management services in accordance with state mandates and agency policies
Collaborate with clinical therapists to support individualized family service plans
Document all services provided and maintain accurate client records in the electronic health record system
Ensure compliance with confidentiality guidelines and proper handling of release of information
Participate in 24/7 on-call rotation as required
Education & Experience Requirements:
Minimum of a bachelor’s degree in Social Work, Counseling, Psychology, or a related field
Must meet Ohio MHAS criteria for Qualified Mental Health Specialist (QMHS) if not licensed
Valid driver’s license and reliable transportation required
Must be insurable under the agency’s commercial liability policy
Experience working with children and families preferred
Must demonstrate knowledge in:
Mental illness and treatment
Individual and group counseling
Behavioral health assessments and crisis intervention
