Job Description

The Case Manager provides direct support services to veterans residing at Chambers Park, an independent living apartment complex serving veterans in Marion, Indiana, to promote housing stability, self-sufficiency, and successful community integration. This position requires an individual who can work independently, demonstrate strong organizational and communication skills, and have experience working in social services and with vulnerable populations.

Key Responsibilities:

  • Provide direct support and case management services to veterans residing at Chambers Park.
  • Assess veteran needs and connect individuals to community resources that promote independence and long-term housing stability.
  • Assist veterans in developing goals that support self-sufficiency and successful community integration.
  • Coordinate and facilitate activities that promote engagement, wellness, and community connection.
  • Document client interactions, services provided, and veteran progress in accordance with program requirements.
  • Assist veterans with transportation needs to access appointments, services, and community resources.
  • Work collaboratively with an interdisciplinary team to ensure coordinated and comprehensive care.
  • Maintain professional relationships with community partners and resource providers.
  • Perform duties independently while demonstrating sound judgment, organization, and effective communication skills.

Qualifications

  • Applicants must have an associate's degree in social services or human services with relevant experience or a Bachelor's degree. A valid drivers license is required.
