Make a Real Difference in the Lives of People Experiencing Homelessness

Are you passionate about supporting individuals and families to overcome barriers and achieve housing stability?

The Salvation Army is seeking a dedicated Case Manager to join our Supported Accommodation team in Caboolture. This rewarding role will see you providing crisis intervention, assessments, case management and practical support to people experiencing or at risk of homelessness. This role will also be required to work on a roster basis at the Peninsula Support Hub also .

Working within a trauma-informed, strengths-based and person-centred framework, you'll empower clients to build resilience, improve social inclusion, secure stable housing and achieve positive long-term outcomes.

About the Role

As a Case Manager, you will provide both phone-based and face-to-face support within a community and residential setting. You'll work collaboratively with clients, internal teams and external stakeholders to deliver high-quality services that make a genuine impact.

Key Responsibilities

  • Conduct client assessments, intake and triage, identifying needs, risks and vulnerabilities
  • Respond to phone, email and walk-in enquiries with timely and effective support
  • Develop and implement individualised case plans with clients
  • Provide ongoing case management and advocacy support
  • Assist clients to access housing, support services and community resources
  • Complete referrals, safety planning and service coordination activities
  • Maintain accurate client records and case notes
  • Participate in case reviews, supervision and local networking opportunities
  • Work collaboratively within a multidisciplinary team to achieve positive client outcomes
  • Contribute to continuous service improvement initiatives

About You

To be successful in this role, you will bring:

  • Tertiary qualifications in Community Services, Social Work or a related field (degree qualifications highly regarded)
  • Demonstrated experience in case management, client assessment and support planning
  • Strong understanding of trauma-informed and person-centred practice
  • Exceptional communication and relationship-building skills
  • The ability to manage competing priorities in a fast-paced environment
  • High emotional intelligence and professional resilience
  • A genuine commitment to empowering vulnerable individuals and families

Essential Requirements

  • Current Queensland Working With Children Check (Blue Card) or ability to obtain
  • Current Australian Driver's Licence
  • Experience working in homelessness, housing, community services or a related human services sector

The Royal Children's Hospital (RCH) has been providing outstanding care for Victoria's children and their families for over 147 years.

We are the major specialist paediatric hospital in Victoria and our care extends to children from Tasmania, southern New South Wales and other states around Australia and overseas.

With a passionate, highly skilled and committed staff campus wide of over 5,000, we provide a full range of clinical services, tertiary care and health promotion and prevention programs for children and young people.

We are the designated state-wide major trauma centre for paediatrics in Victoria and a Nationally Funded Centre for cardiac and liver transplantation.

When it comes to training and research we partner with the very best. Our campus partners, the Murdoch Childrens Research Institute (MCRI) and The University of Melbourne Department of Paediatrics, along with the RCH Foundation, are on site with the hospital in Parkville. In 2016–17, more than 85,654 children attended our Emergency Department, 322,291 specialist clinic appointments were held which was almost 70,000 more than the previous year, more than 17,000 surgeries were performed and more than 48,552 children were admitted to our wards.
