Job Description

The Code Red/Blue Case Manager provides case management, transportation, crisis de-escalation, and overnight support to individuals who are unhoused and accessing emergency shelter. Using a trauma-informed, person-centered, strengths-based approach. Case Manager helps clients meet immediate needs and connect with housing, benefits, healthcare, mental health services, substance use treatment, and community support. The position supports a safe, respectful shelter environment and works under the direction of the Code Red/Code Blue Site Coordinator.

Position Type: Full Time; 36 Hours Weekly

Location: Toms River, NJ

Salary: $42,000 - $45,000

Schedule:

  • Code Blue (November 1st–March 31st): Three assigned overnight shifts per week, from 7:00 p.m. to 7:00 a.m.
  • Code Red (April 1st–October 31st): Five assigned daytime shifts per week, from 10:30 a.m. to 5:30 p.m.
  • Both seasonal schedules include assigned weekends, holidays, and emergency weather activations.

Responsibilities:

  • Engage clients using trauma-informed, person-centered, and culturally responsive practices.
  • Assist with shelter intake, registration, orientation, and identification of immediate client needs.
  • Work with clients to identify priorities and develop practical service plans focused on housing, healthcare, benefits, and other needs.
  • Connect clients with community resources, including housing, benefits, food, healthcare, transportation, and identification services.
  • Assist clients in accessing mental health, substance use, treatment, detoxification, and recovery services.
  • Provide scheduled client transportation while following all agency safety and documentation requirements.
  • Respond to behavioral, medical, and overdose emergencies using trained de-escalation and emergency response procedures.
  • Maintain a safe overnight environment through wellness checks, site rounds, conflict resolution, and prompt reporting of safety concerns.
  • Support shelter operations, including meals, supplies, hygiene needs, sanitation, and morning departures.
  • Maintain accurate documentation, protect client confidentiality, communicate during shift handoffs, and participate in required training and team meetings.

Requirements

  • Bachelor’s degree in Social Work, Psychology, or a related field or
  • High School Diploma or Equivalent and at least three year's of experience providing services to individuals experiencing homelessness.
  • Experience working with individuals experiencing mental health conditions and substance use disorders.
  • Experience in homelessness services, community outreach, case management, or emergency shelter settings preferred.
  • Valid NJ Driver's License with less than 6 points

Since 1959, Bright Harbor Healthcare, formerly known as Ocean Mental Health Services, has been providing counseling to the residents of Ocean County. What was once a single office and two counselors has grown to an agency of over 300 staff, with four outpatient offices, a school, several partial day program locations and many outreach and community programs.
