Job Description

A HOPWA Case Manager contributes to AHF by acting as a friendly, helpful resource to clients and community partners. Clients who qualify for HOPWA services will need assistance to identify and maintain stable housing as well as navigate the process and regulations. As part of their work with community organizations, HOPWA program representatives build and foster relationships with agencies that offer housing and support services.

A HOPWA Case Manager enhances the reputation of AHF by playing an important role in the continuum of HIV care by bridging housing with medical care and other services. Working closely with the Healthcare Center, HOPWA is available to guide the client through the necessary steps for housing barriers.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.

The HOPWA Case Manager will work as a part of a close-knit team to assist individuals with housing, encourage self-sufficiency and provide compassionate and non-judgmental customer service. The HOPWA program staff enables AHF to be of service to the community by providing support to individuals at a critical and potentially overwhelming moment. The ultimate goal of AHF’s HOPWA Program is to assist clients in securing and maintaining stable housing, resulting in improved health outcomes.
