The Case Manager provides comprehensive case management, care coordination, and recovery support services for individuals receiving outpatient treatment. This role serves as a vital member of the treatment team by coordinating client services, facilitating communication between providers and referral sources, supporting admissions and discharge planning, and helping clients access the resources needed to achieve long-term recovery success.
