Provide quality case management as part of multidisciplinary treatment team. Serves as a communication liaison between family and treatment team. Develops and implement discharge plans, ensuring housing, transportation, after care arrangements. Participate in treatment planning meetings, communicating patient and family information, placement and aftercare details.
Must be organized, ability to work well with adolescent families, document proficiently & good phone etiquette

Vista del Mar Hospital is an accredited organization offering comprehensive mental health care, for both adults and adolescents. Our beautiful and spacious facility is located on 16 acres of land in Ventura, California. We are tucked away just off the coast between Los Padres National Forest and the Santa Monica Mountains, slightly northeast of downtown Ventura.
