Your Opportunity

Mission Australia is seeking a compassionate, skilled and motivated Case Manager to join our Clear Minds Program, supporting children, young people and families across the Tweed and Byron Bay regions. This rewarding role is ideal for professionals who are passionate about improving mental health outcomes for vulnerable children and young people and have experience working within trauma-informed, family-centred and strengths-based frameworks.

As a Case Manager, you will deliver early intervention support to children and young people aged 0-18 years who are experiencing, or at risk of experiencing, mental health challenges. You will work closely with families, schools, community organisations and other service providers to provide practical support, therapeutic interventions and coordinated care that helps clients build resilience and thrive.

Based in either Tweed Heads or Lismore, you will provide a combination of school-based, home-based and community outreach support through both short-term and longer-term case management services.

Your key responsibilities will be to:

• Deliver trauma-informed, client-centred case management to children, young people (0–18), and families experiencing or at risk of mental health issues.
• Respond to referrals, conduct initial needs assessments, and provide ongoing support through home visits and outreach services.
• Develop and implement individualized case plans, provide practical support, and facilitate referrals to internal and external services.
• Coordinate and deliver therapeutic and psychoeducational programs, including Play Therapy Groups and mental health education.
• Maintain accurate and timely case documentation, including risk assessments, progress reports, and data entry.
• Work collaboratively with internal teams and external agencies to advocate for clients, participate in case reviews, and support community development.

Requirements for Success:

• Tertiary qualifications and/or relevant experience in the human services industry, with an understanding of mental health.
• Experience working with children, young people and families experiencing mental health challenges, trauma or other vulnerabilities.
• Strong understanding of trauma-informed practice, child and adolescent mental health, and family-focused care.
• Experience delivering tailored case management, including assessment, planning, referral and ongoing support.
• Ability to facilitate individual and group-based interventions, including therapeutic and educational programs.
• Excellent oral and written communication skills, with the ability to maintain accurate case documentation and engage effectively with clients and external agencies.
• Experience working with Aboriginal and Torres Strait Islander peoples and culturally and linguistically diverse communities will be highly regarded
• Current NSW driver’s license
• Current First Aid and CPR Certificate
