Case Manager

The Pilbara Support Network (PSN) is seeking a compassionate and motivated Case Manager to join our team in Newman on a part-time, fixed-term basis for 2 years.

This is a rewarding opportunity to support individuals and families by connecting them with the services, resources and practical assistance they need to achieve positive outcomes. You'll work closely with clients, stakeholders and community organisations to deliver person-centred support while making a genuine difference in people's lives.

About the Role

As a Case Manager, you'll:


  • Build positive relationships with clients and provide ongoing case management support.

  • Undertake client assessments and develop individual support plans.

  • Connect clients with appropriate community services and support networks.

  • Advocate for clients and support them to achieve their goals.

  • Maintain accurate and confidential case notes and client records.

  • Work collaboratively with internal teams, government agencies and community organisations.

  • Monitor client progress and review support plans as required.

  • Contribute to continuous improvement and positive outcomes for the community.

About You

You'll bring:


  • Experience in case management, community services, social services or a similar role.

  • Excellent communication and relationship-building skills.

  • Strong organisational and time management abilities.

  • A compassionate, culturally respectful and client-focused approach.

  • The ability to work independently while contributing positively to a supportive team.

  • Sound computer skills and experience maintaining accurate records.

Desirable


  • Qualification in Community Services, Social Work, Psychology or a related discipline.

  • Knowledge of services available within the Pilbara region.

  • Experience working with Aboriginal communities.
