The Case Manager uses a holistic approach to develop a customized care plan related to health, wellness, finances, living situation, activities of daily living, assistive technology, and life transitions. The case manager continually assesses changes in status and support needs and updates the care plan as needed. The case manager supports clients and families along the continuum of care and makes referrals and linkages with community resources.

Primary Duties

  • Build and maintain long-term relationships with clients, their family members, and other stakeholders (i.e., medical teams, paid and natural supports, legal guardians, etc.).
  • Assess clients’ baseline level of functioning and system of supports.
  • Provide case management through development of a care plan and system for storage and sharing of records; provide updates to team members as necessary.
  • Identify and provide assistive technology supports to maintain independent living and aging in place which may include Installation, training, and monitoring of technology, providing technical assistance, introduction of new technologies that may be of interest to the individual, Coordination of assistive technology assessments/training, and other required documentation to obtain assistive technology.
  • Provide referrals and coordinate services with community resources.

Additional Responsibilities

  • Identify and provide needed medical supports such as:
  • Coordinating, transporting, and attending medical appointments, including follow-up appointments.
  • Providing support with medication management and refills.
  • Developing an emergency response plan and providing backup emergency response support.
  • Identify and provide needed in-home supports which may include:
    Coordination of or providing companion/drop-in supports, and check-ins.
    Coordination of supports for meal preparation, cleaning, and home maintenance.
  • Identify and provide needed social supports which many include:
  • Planning and participation in leisure and recreational activities.
  • Transportation to and from leisure and recreational activities.
  • Identify and coordinate other transportation needs. Provide transportation as identified in the care plan.
  • Identify and provide needed financial and decision-making supports related to:
  • Creating and maintaining budgets.
  • Trustee and/or representative payee services.
  • Coordinating long-term care and insurance options.
  • Conservatorship, guardianship, or power of attorney.
  • Perform other responsibilities as assigned.

Qualifications: Education, Experience, and Certification(s)

  • High school diploma or General Education Diploma (GED) required.
  • Bachelor’s degree in social work, recreation therapy, counseling, or other human services related field required.
  • Valid driver’s license to perform work-related travel and eligible for coverage with ServiceSource’s insurance carrier required.
  • Experience with case management, plan development/implementation, client systems care/coordination, and/or other related job duties required.
  • Master’s degree in social work, recreation therapy, counseling, or other human services related field preferred.
  • Experience with installation and setup of assistive technology (particularly smart home technology) preferred.
  • 2-5 years of experience working with older adults or adults with disabilities preferred.
  • 2-5 years’ experience with case management, plan development/implementation, client systems care/coordination, and/or other related job duties preferred.

Knowledge, Skills, and Abilities

  • Proficiency in Microsoft Office products and the ability to become familiar with ServiceSource specific programs and software.
  • Strong verbal and written communication skills.  
  • Excellent organizational skills.
  • Detail-oriented and able to carry out work with the highest levels of accuracy.
  • Committed to social inclusion model and strengths-based approach.
  • Knowledge of continuum of care and transition planning.
  • Ability to establish and maintain positive working relationships with client systems.
  • Experience writing individual support plans/care plans, progress notes, and discharge reports.
  • Knowledge of Medicaid waiver requirements (CL, FIS, and BI waivers) and WaMS.
  • Knowledge of assistive technology devices/services.

While performing the duties of this job, the employee is regularly required to walk, sit, and stand; reach with hands and arms; and talk, see, and hear. The employee may often or occasionally lift or move durable medical equipment, supplies, or groceries up to 50 pounds. The employee may often or occasionally aid participants with balance (i.e. provide an arm to keep steady) and aid with transferring to and from a wheelchair and/or vehicle. Specific vision abilities required by this job include close vision, distance vision, depth perception, and ability to adjust focus. The ability to walk up and down flights of stairs is required.

Work Environment

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The noise level in the work environment is usually quiet or moderate; however, loud noise may be encountered. Job duties may be required to be performed during inclement weather and during any degree of outdoor temperatures.
