Your Opportunity

The Supported and Safe Integrated Tenancy Program is based at Litchfield Court, a 127-unit social housing complex supporting residents facing a range of complex social and personal challenges. The program provides on-site case coordination and holistic support to individuals and families who are experiencing issues such as family, domestic and sexual violence, alcohol and other drug use, mental ill health, legal and financial stress, and limited access to healthcare. The program works exclusively with Litchfield Court residents, the majority of whom (85–90%) are First Nations people.

This is an on-site position, and our Case Managers use skilled, strengths-based case management approaches to develop personalised, culturally appropriate support plans, conduct follow-up reviews, and coordinate referrals to partner services. The work can involve confronting situations, requiring staff to respond with professionalism, resilience, and the ability to pivot quickly in evolving circumstances. A strong understanding of tenancy and Department of Housing processes is also essential to support residents in achieving safe, stable, and long-term housing outcomes. On-site security is engaged to manage and respond to disruptive behaviours, contributing to a safer and more secure environment for all residents.

Your Key Responsibilities:

  • Receive and respond to referrals from Litchfield Court residents, undertaking assessments using the Risk Assessment Management Framework.
  • Develop holistic, strengths-based culturally appropriate case management plans with residents using SMART goals and coordinate referrals to relevant support services.
  • Provide case coordination and facilitate case conferences with stakeholders to minimise service duplication and improve outcomes.
  • Build respectful, strengths-based relationships with residents and stakeholders, encouraging engagement in service activities, and maintaining strong boundaries.
  • Respond to complex cases or emergencies, supporting individuals and families through crisis and transitions to independence.
  • Maintain accurate, up-to-date records and documentation in line with Mission Australia policies and contractual obligations.
  • Demonstrate child-safe practice by adhering to relevant legislation, policies, and procedures, including proactive risk reporting.
  • Actively promote and participate in service activities and encourage resident engagement and participation.

Requirements for Success

  • Two years of experience in Human Services, a relevant degree, or equivalent qualifications and more substantial relevant experience.
  • Demonstrated experience in applying the Risk Assessment Management Framework in a case management or support role.
  • Knowledge of issues contributing to homelessness such as family violence, mental ill health, overcrowding, and substance use.
  • Cultural competency and experience working respectfully with Aboriginal and Torres Strait Islander peoples, their communities, systems, and protocols.
  • Sound communication and engagement skills, with the ability to build trust and collaborate across diverse individuals and services.
  • Effective time management, written documentation, and IT proficiency, including case note writing and use of Microsoft Office.
  • A resilient, professional approach with a strong sense of boundaries, self-awareness, and commitment to continuous improvement.
  • First Aid Certificate is desired but not required.
  • Full driver licence with no restrictions.

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
