Program Overview:
To keep families together by providing safe, permanent housing, and ongoing accessibility to evidence-based trauma informed support services to Child Protection and Permanency families throughout our region.
Position
Provide support services and case management to families with a history of homelessness or housing instabilityAssistparentsto makea successful transition to self-sufficient living andimprovedindividual and family stabilityProvide coaching and guidance on life skills such as home management, nutrition, budgeting, parenting, education/vocation training, advocacyskillsCo-facilitate groups for parents,childrenand families.Link families to both formal and informal community resourcesWork collaboratively with your KFT peers, DCP&Pandcommunity service providersComplete documentation consistent with program and agency requirementsAs an integrated healthorganizationAcendavalues the use of evidence-based practice or EBP's.
Key Responsibilities:
Requirements:
Bachelor's level practitioner withat least 2years of experiencein socialservice-relatedfield preferred.
Must have andmaintaina valid driver's license with an acceptable driving record
Must be able to exercise independent thinking and good judgment under all circumstances.
Must be able to analyze work, set goals, developplansandutilizetime effectively and efficiently.
