The Case Manager for the EMPOWR Program plays a vital role in improving perinatal outcomes for individuals affected by substance use disorders (SUD). This position provides psychosocial assessments, crisis intervention, resource referrals, care coordination, and ongoing supportive services to enhance patient engagement, retention, and overall treatment success.
Key Responsibilities
Care Coordination & Support• Conduct psychosocial assessments and provide crisis intervention as needed.• Develop individualized treatment and recovery plans in collaboration with patients and interdisciplinary care teams.• Build supportive, trust based one on one relationships that encourage engagement and retention in treatment.• Identify patient needs, strengths, and barriers across domains such as physical health, addiction treatment, social support, occupational goals, financial stability, and housing.
Referral, Resource Navigation & Discharge Planning• Coordinate referrals to inpatient and outpatient treatment programs, rehabilitation facilities, and community based services.• Lead discharge planning to ensure patients transition safely and successfully from inpatient or acute care settings to appropriate follow up services.• Act as an advocate and ally for patients, ensuring they receive appropriate and timely support.
Continuity of Care• Meet patients across various clinical settings, including addiction treatment appointments, medical visits, social service appointments, and therapeutic sessions.• Conduct community outreach to maintain engagement and support patients in real world environments.
Collaboration & Professional Development• Work closely with OB/GYN, Addiction Medicine Providers, Family Medicine, Grants and Research, Behavioral Health Specialist, and other stakeholders to ensure integrated, patient centered care.• Participate in Addiction Medicine clinical meetings, programmatic conferences, and required trainings.
