Job Description

Ohel is seeking Case Managers to provide intensive short-term case management for individuals with Severe Mental Illness that are being discharged following a psychiatric hospitalization. The program supports individuals by assisting them in their transition back into the community. Working on an innovative field-based team, this role will include following up on referrals by contacting hospital social workers, engaging and building rapport with participants while still hospitalized, facilitating optimal discharge plan, ensuring that participants maintain linkages to relevant community providers and resources, and skill development. The Case Manager provides care navigation and fosters connection and engagement with community-based organizations that promote a sense of purpose, physical wellness, education, employment, socialization and community involvement.

Responsibilities:

  • Collaborate with hospital social workers on transition planning
  • Conducting needs assessment to ascertain what the participant requires in order to maintain tenure in the community
  • Accompany clients to and from appointments
  • Assist with appointment navigation and referrals
  • Maintain ongoing communication with collaterals to ensure continuity of care
  • Identifying and utilizing relevant government resources including housing and benefits
  • Keep detailed and comprehensive records of all contact with clients and collaterals

Qualifications:

  • Bachelor’s degree or higher, preferably in a health-related field, or be a New York State Licensed Practical Nurse (LPN)
  • 2 years of case management work experience in a social service agency, preferably serving a behavioral health population
  • Knowledge of mental illness and the needs of individual living with severe and persistent mental illness
  • Knowledge of techniques for identifying risk, including crisis management techniques

The ideal candidate is a self-starter who can work independently as well as within a team and has with excellent follow up skills. This full-time position is located in Brooklyn, offers a hybrid schedule, with inpatient, home, and community visits required. Position includes a competitive benefits package.

Salary: $55,000-$60,000
