Volunteers of America–Greater New York (VOA–Greater New York) is a leading anti-poverty organization serving neighbors in need across New York City, Northern New Jersey, and Westchester. Through 70+ programs, we support over 35,000 adults and children each year, including individuals experiencing homelessness, survivors of domestic violence, youth aging out of foster care, veterans, seniors, and children with developmental delays.

We are currently seeking qualified candidates for the following position:

Position: Case Manager

We seek a dedicated and compassionate Case Manager to join our team in supporting young adults on their journey toward employment and career development. As a Case Manager, you will be vital in providing individualized support, guidance, and resources to help young adults overcome barriers and achieve their employment and training goals. You will work closely with program participants to assess their needs, develop personalized action plans, connect them with relevant services, and monitor their progress throughout the program.

Minimum Qualifications:
• Associate's degree or 60 college credits courses in Human Services or related field or High School Diploma or GED with 2 years relevant experience.
• Knowledge of youth development principles, career counseling techniques, and employment trends.
• Excellent organizational skills with the ability to prioritize tasks, manage time effectively, and meet deadlines.
• Ability to work independently as well as collaboratively within a multidisciplinary team environment.
Case Manager Principal Responsibilities:
1. Publicize and market the program to potential clients. Outreach to local community-based organizations, community boards, churches, and other organizations to build a referral source.
2. Conduct comprehensive assessments of young adult’s strengths, needs, skills, and career goals to develop individualized service plans.
3. As needed, assist participants in accessing supportive services such as transportation, housing assistance, healthcare, childcare, and mental health counseling.
4. Monitor participants’ progress in completing training programs and obtaining employment. Advocate on behalf of the participants to ensure their needs are met and barriers are addressed effectively.
5. Maintain accurate and confidential participant records, documentation, and reports according to program guidelines and regulations.
6. Participate in teams’ meetings, training sessions, professional development, and employment services.
7. Contribute to program evaluation efforts by collecting data, conducting surveys, and providing feedback to inform program improvements and best practices.
8. Perform follow-up/retention services for participants who have completed the program.
