About Heartview Foundation:
Heartview Foundation is North Dakota’s most experienced private, non-profit alcohol and drug treatment facility. Since 1964, we have helped thousands of individuals and families find recovery through evidence-based treatment, compassionate care, and comprehensive support.
The Case Manager supports the treatment team by coordinating care and services that align with each patient’s treatment goals. This role involves facilitating referrals, assisting with discharge planning, and connecting patients with community resources to promote long-term recovery. The Case Manager ensures all services meet licensing, ethical, and evidence-based standards.
