Job

Summary:
As the Case Manager you will support the Manager for the Transitional Housing Program and program residents to ensure the coordination, 24-hour operation, and program implementation of the Transitional Housing Program (THP).

Supervisory Responsibilities:
- None
Duties/Responsibilities:
- Responsible for promoting and providing a residential program environment that coincides with the YWCA Mission.
- Develops goals and outcomes with individual residents.
- Provides case management support for individual residents as needed, assists with problem solving, and provides support for health, legal, and advocacy issues.
- Assists with Department of Transitional Assistance (DTA) benefits, housing and Mass Health applications.
- Interfaces with all YWCA departments, advocating for residents’ needs.
- Works with housing providers and community agencies to enhance program services and program participants’ access to community resources.
- Maintains residence database and prepares program reports.
- Performs other duties as requested by Manager.

QualificationsRequired Skills/Abilities:
- Demonstrated skills in program management, supervision, and financial management.
- Experience in the field of Substance Abuse Treatment and Recovery preferred.
- Bi-lingual (English/Spanish) preferred.
- Demonstrated ability to exercise good judgment.
- Experience working with diverse populations, such as women and their families and communities of color.
- Ability to work with community groups and volunteers.
Education and

Experience:
- High school diploma required.
- Bachelor’s Degree in related field such as Human Services, Counseling, or Social Services or at least 5 years of related experience.
- Must pass criminal background check.
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.
