Case Manager

Boone Memorial Health is seeking a compassionate and highly organized Case Manager to support clients engaged in behavioral health and substance use disorder programs. This role guides individuals through assessments, treatment coordination, crisis response, and connections to needed services. The Case Manager serves as a central point of support, ensuring clients receive timely, appropriate, and coordinated care.


Key Responsibilities

Complete Mental Health Assessments by Non-Physician to determine client needs, presenting problems, and functional status. Administer screening tools, data collection instruments, and agency-required assessments. Conduct intake interviews and gather psychosocial history. Complete DUI program enrollments and ensure all required documentation is accurate and complete. Educate clients on DUI program expectations, timelines, and compliance requirements. Develop care recommendations based on assessment outcomes. Meet regularly with clients to monitor progress, identify changes in needs, and support adherence to MAT and behavioral health services. Coordinate and facilitate referrals to medical care, social services, housing, employment, and recovery supports. Advocate for client access to needed services and assist in removing barriers. Educate clients about available benefits, treatment expectations, and patient rights. Provide crisis intervention, including de-escalation, safety assessment, and coordination with emergency services when needed. Maintain accurate and confidential documentation meeting all agency, Medicaid, and HIPAA standards. Enter assessment data, service contacts, and outcome measures into program systems. Participate in interdisciplinary team meetings and work closely with clinicians, prescribers, peer support professionals, and community partners. Ensure program compliance with MAT guidelines, state regulations, and organizational policies.



Qualifications


Bachelor’s degree in Counseling, Social Work, Psychology, or a related human services field. Experience working with individuals facing mental health conditions, substance use disorders, trauma, or related challenges. Experience with data management, case management software, EHRs, or reporting systems. Knowledge of confidentiality and regulatory standards (HIPAA, 42 CFR Part 2). Strong understanding of community resources and referral processes.

Physical Demands

Ability to sit or stand for extended periods, walk, lift up to 25 lbs., climb stairs, bend, reach, twist, push, pull, grasp, and perform repetitive arm–hand movements.

Travel

Minimal travel required.
