As a Case Management Supervisor at Horizon, you will….

  • Supervise and support TCM and PRS staff through regular coaching, training, and performance management.
  • Manage referral management and oversight
  • Monitor caseloads, service delivery, productivity, and documentation quality.
  • Ensure compliance with CCBHC, OMH, OASAS, Medicaid, and agency standards.
  • Review documentation and conduct quality assurance activities.
  • Assist staff with complex cases, care coordination, and barriers to engagement.
  • Collaborate with internal teams and community partners to ensure continuity of care and successful service outcomes.
  • Participate in program development, quality improvement initiatives, and regulatory audits.
  • Prepare reports and maintain program performance data.
  • Service delivery to patients as needed.

What makes you a great candidate?

We can’t wait to learn more about you! Here are a few specifics of what you’ll need for the job:

  • Bachelor's degree in Social Work, Human Services, Psychology, Rehabilitation Counseling, or a related field required.
  • 2 years of experience in behavioral health and vocational required.
  • 2 years of supervisory or leadership experience required.
  • Knowledge of behavioral health, recovery-oriented care, and trauma-informed practices.
  • Understanding of Medicaid-funded services and community support systems.
  • Strong leadership, communication, organizational, and problem-solving skills.
  • Proficiency in electronic health records and Microsoft Office applications.

Location:

This position will be split between two locations: 637 Davison Road, Lockport NY and 2400 Pine Ave, Niagara Falls NY. Travel between the two sites may be required.

Hours:

Monday - Friday: 8am - 5pm

Compensation:

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. This position starts at a salary of $64,000-$66,000 annually. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

This position entails eligibility for a sign-on bonus of up to $1,500 for your commitment to work at our Horizon Certified Behavioral Health Clinics (CCBHC) located at our Pine Avenue Recovery Center and Lockport Recovery Centers for at least one year. A change in scheduled hours or location of employment may impact eligibility of bonus.

Physical demands associated with this position include:

  • Sitting for extended periods, alternating between standing and sitting
  • Mobility required with repetitive wrist, hand, and finger movements
  • Handling light duties, occasionally lifting objects up to 20 pounds
  • Local travel between worksites
  • Exposure to various environmental conditions including low/high temperatures, outdoor elements
