
The Case Management Engagement Specialist is responsible for engaging individuals with serious and persistent mental illness who are disconnected from traditional services or at risk of disengagement. The specialist builds trust with clients and their families, assesses needs, creates treatment plans and connects them to appropriate mental health, medical, housing, and social services. They may also collaborate with schools and DCYF. This role focuses on outreach, early intervention, and reducing barriers to care, with a strong emphasis on person-centered approaches. The ability to multitask, manage time well and collaborate with other staff is required for this position.
