JOB SUMMARY:
The discharge planning assistant is responsible for providing accurate and timely administrative and technical support to the
discharge planning and utilization management teams, helping ensure a positive patient experience, effective coordination of
care, utilization review processes, and compliance with both hospital policy and CMS guidelines.

QualificationsEducation/Certification:  Associate degree or 2 additional years of experience in lieu of a degree, with high school diploma or equivalent.
Equipment/Technology:  Basic computer knowledge.
Language/Communication: Effective written and oral skills. Ability to comprehend and follow detailed instructions. Ability to effectively present information and respond to questions from patients, staff members and community partners. Ability to speak clearly and obtain necessary information from patients, to listen and seek clarification when needed for accurate dispersal of information.
Mental Capabilities:  Ability to organize, recognize and correct inefficiencies, handle and prioritize multiple requests
