PAMF-Palo Alto Medical Foundation ALDThe Assistant Care Manager 3 works in collaboration with the Physician, licensed Psychotherapist, consulting Psychiatrist, and other care team members to assure the appropriate level of care and coordination for outpatient services.
The Assistant Care Manager reports to the Healthy Young Minds Clinical Manager. The Assistant Care Manager has frequent contact with patients, families, physicians, the interdisciplinary team, schools, ancillary services, and community resources.
In addition to meeting with patients and families, this position is responsible for gathering and reporting patient functioning data and clinical measurements, utilizing the patient care registry, communicating between collaborative care team members, direct patient contact to assigned patients and family members, and direct intervention with patients and families, to include crisis intervention and de-escalation where required.

Pay Range is $35.35 to $45.95 / hour
The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
