Job Description Summary:

Supports safe, timely, and efficient patient discharges and care coordination by facilitating communication, scheduling, and collaboration among patients, families, and multidisciplinary care teams.

Job Description:

Essential Functions:

• Assists with the scheduling of various inpatient and outpatient services, including tests, procedures, and follow-up appointments.
• Communicates with patients and families regarding admissions and appointments and connects them with appropriate resources.
• Collaborates with multidisciplinary teams to ensure cohesive care planning and effective communication throughout the discharge process.
• Obtains required information from patients, caregivers, healthcare providers, and other external teams.
• Assists with completing required documentation such as FMLA forms and obtaining payer authorizations for medications, equipment, and services related to post-discharge care.
• Monitors related metrics and assists with data gathering to support process and patient care improvements.
• Ensures compliance with regulatory and organizational privacy and safety standards.

Education Requirement:

Associate's Degree, or equivalent education and experience, required.

Skills:

• Knowledge of current medical terminology.
• Knowledge of payer negotiations and authorization requirements.
• Excellent communication and interpersonal skills.
• Able to work in a fast-paced clinical environment and manage multiple tasks effectively.
• Demonstrated effectiveness in working with multi-disciplinary teams.

Experience:

• Experience in an inpatient or outpatient clinical setting, preferred.
• Experience with electronic medical records, preferred.
