Summary
The Case Finder/Intake Coordinator represents TKRH within the local and regional healthcare community, cultivating strong relationships with hospitals, physicians, payor sources, and other referral partners. This position blends clinical knowledge with outreach and marketing responsibilities to increase awareness of TKRH services and support appropriate admissions.
It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support the organization strategic plan, the goals and direction of the quality assurance performance improvement process, and activities.
QualificationsKey Responsibilities:
- Act as primary representative and marketing ambassador for TKRH across the healthcare community.
- Develop, strengthen, and maintain relationships with referral sources including hospitals (local and out-of-state), physicians, and payor partners.
- Educate referral and payor sources about TKRH's specialized programs: acute pediatric rehabilitation, Totally Infant Program (TIP), pediatric subacute services, intermediate care, and outpatient therapy.
- Coordinate and participate in marketing activities including distribution of materials, tours, lunch-and learns, open houses, and outreach.
- Collaborate with Case Management and leadership to support admissions and census growth.
- Perform onsite clinical evaluations and assist with admissions.
- Provide timely communication and follow-up with referral partners and internal teams.
- Track referral outcomes and report trends and opportunities.
- Experience in healthcare outreach, liaison roles, intake, case management, or marketing highly valued.
- Clinical background preferred (RN, LVN, PT/OT/ST, or related).
- Strong communication and relationship-building skills.
- Ability to work independently, prioritize, and represent TKRH professionally.
- Willingness to travel for evaluations and outreach.
