Job Shift: Flexible/Non-TraditionalThis position is responsible for care coordination for youth and their families to ensure linkage and engagement with mental health/substance abuse treatment that supports the client’s goals and to provide outreach to children and their families. Services shall include education, identification, and linkage with community resources, as well as follow-up to ensure the client is successfully engaging in the services. This position will encourage, educate and engage prospective clients related to the importance of wellness, treatment and recovery relevant to their current needs and functioning.
Facilitates all necessary meetings for youth in the admission and discharge processes.
2. Obtains all needed signatures from parents/guardians for legal/admission paperwork, service plans, safety plans, updated consents, and other relevant documentation.
3. Works with the team to create and update all client and family centered service plans, assuring that all plans meet Camelot policies, state and federal law/rules, funding requirements and COA standards in a timely manner.
4. Conducts referrals and follow-up to ensure client linkage, engagement in and satisfaction with services.
5. Conducts regular face-to-face and virtual contacts with children, parents, and others for their caseload.
6. Reviews and assesses the family’s progress made toward connection and engagement in services.
7. Maintains consistent communication with families regarding clients’ functioning in the home, school, and any service agencies in order to maintain continuity of care.
8. Establishes and maintains relationships with community resource providers, particularly emphasizing facilitation of the referral process and providing continuity of care to the families on the caseload.
9. Establishes and maintains relationships with children and families including awareness of and attention to achievement of improved functioning, satisfaction with services, and overall safety of clients.
10. Collaborates with and helps to guide the Family Peer Support Specialist
11. Maintains electronic and paper client files according to company policy, established timeframes and applicable regulations.
12. Accurately completes case notes on a continual basis which are factual, up to date and descriptive. All case notes are entered into the required data system according to current policy and contractual outcomes.
13. Attends individual case staffings, community meetings and discharge planning.
14. Maintains regular contact with referral sources and other stakeholders including the funder, the school system, the Department of Juvenile Justice, and the families of clients being served.
15. Works with community stakeholders to identify service gaps and opportunities to improve service delivery in the community.

QualificationsMaster's degree preferred in a Human Service discipline from four-year accredited college or university and experience working with children/adolescents related to their therapeutic/treatment needs
