Provides assistance in participants daily routine, activities and personal care at their place of residence.
RESPONSIBILITIES
Patient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands
Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly
Documentation. Accurately and completely document client care on activity sheet
Compliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements
REQUIRED QUALIFICATIONS:
HHA Certified or Ability to meet one of the following eligibility requirements for Competency:
Currently certified CNA along with completion of written test and one day HHA Skills competency class within 30 days of hire.
Documented home health aide or nurse aide training and competency evaluation from an out-of-state training program
The Certified HHA who wishes to be employed after a two-year lapse in employment as a HHA must take the 1 Day Skills Competency Class. They must complete a written test and pass the one day HHA Skills competency class within 30 days of hire and before working as an HHA. If the candidate has worked as a Certified HHA within the past 24 months, and is actively certified, no class is needed.
Nursing student who has documented evidence of successful completion of course work requiring mastery of home health aide tasks within the past 24 months-Fundamentals of Nursing and must show completion of written test and one day HHA Skills competency class within 30 days of hire.
Veteran who was trained in the United States Military as medical technician or medic must show completion of written test and one day HHA Skills Competency Class within 30 days of hire.
PREFERRED QUALIFICATIONS:
High school diploma or equivalent
For ElderONE employment and other areas of elderly care: 1 year experience working with the frail elderly population
Compassionate, patient focused and dependable professional
Critical thinking skills, decisive judgment and the ability to work with minimal supervision
Exceptional documentation and communication skills
Ability to follow verbal and written instructions

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
