Clinical Opportunities:

Hospital Liason: $68,000 - $84,000/ year

LPN Resident Care Supervisor: $27 - $35.34/ hour

Resident Care Aide: $16.50 - $18.02 / Hour

Non Clinical Opportunities:

Activity Leader: $15.60 - $18.31 / hour

Chef/ Cook : $18 - $22 / hour

Responsibilities

Clinical Opportunities:

  • Hospital Liason
    • As a Hospital/Clinician Liaison, you will play a vital role in ensuring smooth transitions of care by screening prospective residents for designated Elderwood facilities. You will serve as a trusted representative of multiple sites, developing a deep knowledge of their services, environment, and operations. In this role, you’ll act as both a clinical resource and a marketing ambassador, helping strengthen Elderwood’s reputation as a partner of choice for hospitals, clinicians, and the communities we serve. This position reports directly to the executive team
  • LPN - Resident Care Supervisor
    • The Resident Care Supervisor (LPN) for Assisted Living Facilities (ALF) assists with ensuring the general health and well-being of our residents by providing nursing care. This position is responsible for assisting with medications and monitoring the personal care services provided by the Resident Assistants; and reporting care and change in condition of residents to staff of next shift and the Resident Care Manager.
  • Resident Care Aide
    • Resident Care Aide - Personal Care Aide (PCA) team members assist with ensuring the health and well-being of our residents by providing personal care support. This position is responsible for meeting the physical and emotional needs of residents under the direct supervision of a licensed nurse ensuring that the highest degree of quality resident care is maintained at all times.

Non Clinical Opportunities:

Activity Leader:

  • As an Activity Leader, you will plan and lead a variety of fun and enriching recreational activities tailored to residents’ interests and abilities. Your efforts will contribute directly to enhancing quality of life and building community within our facility.

Chef

  • Meal times are highlights of daily life, and excellent nutrition and appetite appeal are central to the well-being of residents and patients. Our chefs, wait staff and kitchen staff prepare and serve delicious, wholesome menus. Cook for our residents with weekly rotating menus. Work from our recipes.

Server

  • ervers provide restaurant quality service and assist with ensuring the health and well-being of our residents by being responsible for table service. This individual is a key member of our dining experience and contributes to various aspects of the dining room/kitchen functions.

Qualifications

  • Clinical Based Positions: Credentials verifying appropriate certificate and license in New York State- LPN, PCA, HHA,e.t.c.: current State license/Certificate required.
  • Ability to communicate well verbally and in writing required.
  • Qualities of maturity, diplomacy, and ability to work well with others required. Caring, respectful attitude toward residents required.
  • Physical stamina for constant activity required.
  • Ability to perform tasks to established standards of excellence required.
  • Demonstrated willingness to uphold the vision and mission of the company by following the core values of Integrity, Teamwork, and Passion for Excellence and Reverence for the Individual is required.
  • These position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
