Trillium Health Resources has a career opening for a Care Transitions Care Manager to join our team! The Care Transitions Care Manager provides comprehensive care coordination to support individuals transitioning between care settings and maintaining stability over time. This role assesses needs, develops care plans, and coordinates services across providers and community resources.
This position will serve as a member of the State Hospital integrated care team for the State Psychiatric inpatient setting. The Care Transitions Care Manager is expected to assess and respond to members’ health and safety when the Trillium Catchment is under a state of emergency or weather advisory.
