As the Care Team Supervisor, you’ll contribute to our success in the following ways:
- Coordinate and schedule homemaking, personal care, and home health aide services in a timely manner, including coverage for planned and unplanned absences
- Maintain accurate client service records, plans of care, caregiver schedules, and required documentation
- Review and manage contract-specific documentation and ensure all required authorizations align with scheduled services
- Educate clients on services, policies, and Federal/State regulations to ensure understanding and compliance
- Support branch growth initiatives and operational planning efforts
- Review reports and monitor systems to ensure accuracy in billing, payroll, utilization, and service delivery
- Process and manage employee timesheets and collaborate with payroll and timekeeping teams as needed
- Ensure ongoing compliance with all State, Federal, licensure, contract, and regulatory requirements
- Provide guidance, coaching, support, and performance counseling to caregivers to promote engagement and quality care
- Ensure caregiver compliance with required trainings, certifications, medical requirements, health assessments, and in-services
- Conduct caregiver orientations and maintain complete caregiver and client documentation records
- Monitor daily client service delivery requirements to ensure adequate caregiver availability and shift coverage
- Review open shifts, utilization reports, and staffing needs daily to maintain continuity of care
- Submit employee incident, allegation, and workers’ compensation information to HR in a timely manner
- Communicate professionally and effectively with caregivers, clients, case managers, supervisors, and leadership teams
- Represent Elara Caring in a professional and positive manner within the community and through participation in local organizations and initiatives
- Remain flexible and available to support direct care services as needed
- May oversee and provide guidance to Chinese-speaking supervisors to ensure alignment with branch and organizational objectives
- Support Elara Caring’s mission, values, and commitment to delivering exceptional patient-centered care every day
- Perform additional duties and projects as assigned to support team and organizational success
What is Required?
- High School Diploma or GED required
- Associate’s or Bachelor’s degree preferred
- Experience working with elderly or disabled populations preferred
- Five (5) years of experience in a fast-paced office or healthcare environment preferred
- Working knowledge of Medicare/Medicaid and home care operations preferred
- Knowledge of kidney-related diagnoses, including dialysis treatments and post-dialysis care needs is highly preferred
- Strong computer proficiency and documentation skills
- Excellent organizational and multitasking abilities
- Professional verbal and written communication skills
- Strong leadership, coaching, and motivational skills
- Ability to thrive in a fast-paced, high-volume environment
- Bilingual fluency (Spanish, Mandarin/Cantonese, Russian, or Portuguese) may be required based on location
- Reliable personal transportation and ability to travel locally and occasionally to support audits or operational needs
