Jefferson Healthcare is seeking a highly organized and detail-oriented individual to join our Specialty Clinic as a Care Team Specialist II. The ideal candidate will possess excellent communication skills and the ability to work well with both patients and healthcare professionals. The primary responsibilities of this position will include managing patient referrals, ensuring that all necessary information is obtained and communicated to the appropriate parties, and maintaining accurate records of all referrals. The successful candidate will also coordinate and schedule appointments, follow up with patients and physicians, and provide excellent customer service to all parties involved.
