
Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing, assisted living facilities, personal care homes, and independent living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Psychologists, Licensed Clinical Social Workers, and Clinical and Administrative Support Staff. In this role, the Care Plan Coordinator will be responsible for collaborating with providers in creating a comprehensive Care plan for use with Chronic Care Management (CCM) or Advanced Primary Care Management (APCM).
