As a Care Partner you will be providing Older Adults, their families and carers with access to responsive care and case management, ensuring the customers receive services and support tailored to their needs and requirements.
On a typical day you will liaise with older persons, either on phone or in their home, to follow up on their care requirements. Your conversations will revolve around their goals to achieve happy, fulfilled lives whilst remaining independent in their own homes.

If you’re an experienced Case Manager / Care Partner, qualified in Community Services, Health, Aged Care or related discipline and enjoy assisting older adults with their day to day needs through accessing quality care providers, you might be exactly the person we’re looking for.
