Care Partner – Uniting Home & Community Care
Southwest Sydney

This is more than case management, it’s about building relationships, advocating for choice, and coordinating care that genuinely reflects each person’s goals, preferences and life.

About the role
You’ll be the central point of contact for your clients, supporting them to access the right services and get the most out of their funding, while staying connected to their community.
Your day-to-day will include:
• Managing a caseload and leading person-centred care planning
• Coordinating services tailored to each individual’s needs and budget
• Building strong relationships with clients, families, and their Circle of Support
• Monitoring progress and adjusting care plans as needs change
• Identifying and escalating risks, including vulnerable or complex cases
• Collaborating with internal teams and external providers to deliver seamless care
• Supporting clients to remain independent and socially connected
About You
You’re someone who genuinely puts people first and is confident balancing multiple priorities in a fast-paced environment.
You’ll also have:
• Experience in aged care, community services, or case management
• Strong communication skills and the ability to build trust quickly
• Experience coordinating services and managing competing priorities
• A solid understanding of person-centred care
• Confidence working with systems and maintaining accurate documentation
• Ability to manage budgets and support clients to understand their funding
Essentials:
• Certificate IV in Aged Care, Community Services, Healthcare (or similar)
• Current First Aid Certificate
• Current and unrestricted Driver’s Licence
Even better if you have:
• Experience with client management systems
• A Diploma or Bachelor qualification in a relevant field
• Your own vehicle and comprehensive insurance
