Are you passionate about supporting older people to live independently, safely and connected to their community?
Uniting is seeking an experienced Neighbourhood Care Partner to join our Home and Community Care team. You will be responsible for case management, care planning and coordinating person-centred services that support clients to remain living in their own homes for longer.
As a key point of contact for clients and their support networks, you will ensure services are tailored to individual needs, preferences and available funding.
