Job Description

Location: Aveo Retirement Communities - Northern Gold Coast & Banora Point

Opportunity: Permanent full-time | Monday-Friday

Reports to: Regional Home Care Manager

Salary: ~$107K + super + Aveo vehicle (with fuel card)

The Opportunity

As a Care Partner at Aveo, you’ll be a trusted partner and familiar face to our residents, helping them navigate their home care options and maintain their independence for as long as possible. Based within our retirement communities, you’ll build genuine connections, develop personalised care plans and coordinate services that make a real difference in residents’ lives.

If you’re AHPRA-registered with a background in Nursing or Allied Health (e.g. Physiotherapy) and experience managing a caseload, we’d love to hear from you! Experience with the Support at Home program (formerly Home Care Packages) is a bonus.

Your Impact

  • Meet with prospective residents and families to assess care needs and explain funding options clearly and compassionately
  • Develop personalised home care plans and ensure services are in place as required
  • Coordinate with external service providers including GPs, Allied Health and other care and support services
  • Monitor resident wellbeing over time and adjust care plans as needs evolve
  • Guide residents and families through the My Aged Care and Support at Home journey, providing excellent customer service at every moment
  • Maintain accurate documentation and records via our Care Management System (Alayacare)
  • Promote Aveo Care at Home services by attending community events and delivering information sessions to residents

What you'll bring

  • Current AHPRA registration with prior experience in the home care or community care space
  • Knowledge of Support at Home program (or HCP, NDIS, DVA or CHSP) and/or My Aged Care and ACAT
  • Confidence using clinical judgement in an autonomous, community-based setting
  • Excellent communication skills - with residents, families, and multidisciplinary teams
  • Confident using a Care Management System (e.g. Alayacare or similar), Microsoft Office and proficient with technology
  • Drivers License

Why Aveo?

  • Monday-Friday hours - no weekends or night-shifts
  • Spend your day within our Aveo communities and with consumers, rather than your day being spent travelling on the road
  • A sense of genuine belonging as you'll become a familiar face not just to your consumers, but to the entire retirement community around you
  • Everything you need from day one - Aveo vehicle (with fuel card), laptop, phone and all systems training provided
  • A dedicated team behind you to provide onboarding training, ongoing support and strong systems from our highly experienced Care at Home team
  • Health and well-being benefits including paid leave days, health insurance, retail and leisure discounts, plus annual flu vaccinations and skin checks
  • Employee Referral Program and Employee Assistance Program
