Wyanga Aboriginal Aged Care is an Aboriginal Community Controlled Organisation supporting Elders to live independently, safely and with dignity on Country.
Our work is grounded in culture, connection and community. We walk alongside our Elders, providing support that respects who they are, where they come from and what matters to them.
As a Care Partner, you’ll build trusted relationships with Elders and work alongside them to understand their needs, goals and preferences.
You’ll manage a client caseload, develop and review practical care plans, and coordinate supports that help Elders remain independent and connected at home. You’ll monitor how things are going, respond to changes early and ensure services remain appropriate to each person’s goals and available funding.
Working with families, authorised supporters, the Wyanga team and external providers, you’ll help Elders navigate their care and maintain continuity of support.
You’ll bring experience in aged care, community services or a related field, with the confidence to manage a caseload and follow things through.
You are someone who:
Experience in care planning, case management or service coordination will be highly regarded.
Please submit your resume and a short cover letter outlining your relevant experience and why you would like to join Wyanga.
Aboriginal and Torres Strait Islander people are strongly encouraged to apply. We also welcome applicants who bring relevant experience and a demonstrated commitment to culturally safe practice.
