About the Role

As a Care Partner, you will be the trusted point of contact for a portfolio of clients, coordinating person-centred home and community care that reflects their goals, preferences and funding. You will build strong relationships, develop responsive care plans and bring together the right services so clients can remain safely at home and connected to their community for longer.

Your Key Accountabilities:

  • Build trusted relationships with clients and ensure their voice, choices and goals guide every aspect of their care.
  • Develop, review and coordinate individual care plans that meet client needs within their available funding.
  • Act as the central point of contact for clients, families, service providers and internal teams.
  • Recognise and escalate changing needs, risks and vulnerabilities, including complex care planning requirements.
  • Work collaboratively across Home and Community Care to deliver safe, seamless and responsive services.
  • Contribute to quality improvement, governance and risk management activities.
  • Maintain accurate records and confidently use client management systems and other practice tools.
  • Travel between Uniting community sites and client locations as required.

What You’ll Bring:

  • Certificate IV in Aged Care or a Diploma of Community Services. A diploma or bachelor qualification in a relevant area such as aged care, healthcare or business will be highly regarded.
  • Experience coordinating care, managing a client portfolio or working in community-based services.
  • Strong relationship-building, communication, organisation and problem-solving skills.
  • Confidence using client management systems and maintaining accurate records.
  • A current Australian driver’s licence, access to your own vehicle and comprehensive car insurance.
  • A current First Aid Certificate.
