Your opportunity

We are seeking a Relief Care Partner to join our team on a full‑time, 12‑month fixed term contract. Our office is based in Macquarie Park and you'll be supporting home care clients across Northern Sydney and Northern Beaches Region.

This relief role provides essential coverage during periods of leave, vacancies and increased demand, offering variety across different client portfolios and locations. Flexibility and adaptability are key, as you step in to ensure continuity of care and positive outcomes for our clients.

As a Relief Care Partner, you will provide high‑quality, ethical and sustainable care coordination, supporting older people to live safely and independently in their own homes.

You’ll work closely with clients, families, care staff, clinicians, health professionals and service providers to ensure care is delivered seamlessly and in line with individual needs.

Key responsibilities

With empathy, professionalism and integrity, you will:

  • Provide relief case management across multiple client portfolios as required

  • Conduct client check‑ins, reviews and assessments to ensure wellbeing and safety

  • Support the delivery and coordination of services in line with Care Plans and budgets

  • Maintain accurate records, documentation and client notes

  • Liaise with clients, families, care staff and external providers to ensure continuity of care

  • Escalate risks, incidents or concerns appropriately to support safe service delivery

  • Ensure services comply with Aged Care Quality Standards, program guidelines and Anglicare policies

  • Support care staff through clear communication and feedback when stepping into portfolios

What you’ll bring

You are adaptable, organised and compassionate, with the confidence to quickly build rapport with clients and teams. You thrive in a fast‑paced, varied role and are comfortable stepping into new situations.

You will also need:

  • Previous experience as a Care Partner, Case Manager or similar role (essential)

  • Certificate IV in Aged Care, Community Services or equivalent (minimum)

  • Current driver’s licence and access to a reliable vehicle

  • Experience working in Home Care, Community Services or Aged Care

  • Intermediate computer skills (MS Office, client management systems)

  • Strong communication, time management and problem‑solving skills

Desirable:

  • Diploma in Case Management or related field

  • Experience managing client budgets and service coordination

  • Knowledge of Aged Care Quality Standards

The Royal Children's Hospital (RCH) has been providing outstanding care for Victoria's children and their families for over 147 years.

We are the major specialist paediatric hospital in Victoria and our care extends to children from Tasmania, southern New South Wales and other states around Australia and overseas.

With a passionate, highly skilled and committed staff campus wide of over 5,000, we provide a full range of clinical services, tertiary care and health promotion and prevention programs for children and young people.

We are the designated state-wide major trauma centre for paediatrics in Victoria and a Nationally Funded Centre for cardiac and liver transplantation.

When it comes to training and research we partner with the very best. Our campus partners, the Murdoch Childrens Research Institute (MCRI) and The University of Melbourne Department of Paediatrics, along with the RCH Foundation, are on site with the hospital in Parkville.
