
Wyanga Aboriginal Aged Care is an Aboriginal Community Controlled Organisation supporting our Elders to live independently, safely and with dignity on Country.
Our work is grounded in culture, connection and community. We don’t just deliver services - we walk alongside our Elders to ensure they are supported in a way that respects who they are, where they come from, and what matters to them.
This role sits at the heart of how we support our Elders to remain independent, safe and connected at home.
As a Care Partner, you’ll work closely with clients to understand their needs, goals and what matters to them, and translate that into clear, practical care plans that support independence and wellbeing.
The role focuses on care planning and case management - actively monitoring how clients are going, identifying changes early, and making sure supports remain appropriate, effective and aligned to their goals and available funding.
You’ll build trusted relationships with clients and their families, providing guidance and continuity, while working alongside the broader team to ensure services are delivered in a way that makes sense for each person.
It’s a role suited to someone who brings strong judgement, a person-centred approach, and the ability to work independently while staying connected to a team environment.

In 2016–17, more than 85,654 children attended our Emergency Department, 322,291 specialist clinic appointments were held which was almost 70,000 more than the previous year, more than 17,000 surgeries were performed and more than 48,552 children were admitted to our wards.
