We're looking for someone with case management experience to provide support to our Support at Home participants and their supporters.
The Care Partner plays a vital role in providing high quality, relationship-based, person-centred in-home case management, tailored to each individual’s needs, goals and preferences.
The Care Partner builds trust and rapport with their participants and supporters by working collaboratively to gain a deep insight into age-related health needs within the context of the participant’s life. This relationship-based support and planning underpins every care management
activity completed by the Care Partner, driving improved outcomes, continuity of care and participant satisfaction.

This is a permanent full-time role based out of our Swan Hill office.
This position has been classified under the Social, Community, Home Care and Disability Services Award as Level 4 with a starting annual salary of $88,090 + Superannuation + Salary Packaging.
