Are you a compassionate healthcare professional who enjoys building relationships, advocating for patients and families, and helping people navigate their healthcare journey?

Advantage is seeking a Care Navigator to support patients and families across our continuum of care, helping ensure they receive the right care, at the right time.

This unique role will support smooth transitions between rehabilitation, home health, and hospice services while serving as a trusted resource for patients, families, referral sources, and healthcare professionals.

Responsibilities

What You'll Do:

  • Serve as a patient and family advocate throughout their healthcare journey
  • Educate patients and caregivers about available services, eligibility, goals of care, and transition options
  • Help identify when patients may benefit from transitioning between rehabilitation, home health, and hospice services
  • Coordinate seamless handoffs between service lines to help reduce gaps in care and avoid unnecessary hospitalizations
  • Build trusting relationships with patients, families, physicians, referral sources, and community partners
  • Collaborate with nurses, therapists, social workers, case managers, physicians, and clinical leadership
  • Provide compassionate guidance and support during changes in health status and progression of illness
  • Monitor patients who may be at risk for decline or hospitalization and communicate concerns to the appropriate clinical team
  • Support a positive patient and family experience throughout the continuum of care

As a Care Navigator, you will have the opportunity to make a meaningful impact by connecting patients and families to the appropriate level of care while working alongside a supportive interdisciplinary team.

What We're Looking For:

  • Healthcare professional preferred, including a Social Worker or Skilled Nurse
  • 2–3 years of experience in home health, hospice, rehabilitation, case management, care coordination, or post-acute care
  • Strong communication, relationship-building, and organizational skills
  • Ability to manage multiple patient needs, calls, and priorities effectively
  • Knowledge of Medicare guidelines and post-acute care transitions preferred
  • Experience with electronic medical records and documentation systems
  • Valid driver's license and reliable transportation
