Position Overview:

We are seeking a highly organized, detail-oriented, and dependable Intake Care Manager to support the successful activation of new clients and caregivers across Ohio. This is a field-based role focused on completing new-client activation visits, collecting required documentation, conducting BCI/FBI fingerprint scans, and ensuring each applicant and family successfully completes the onboarding process.

The Intake Care Manager will serve as the first in-person point of contact during the beginning of the caregiver journey. This role does not manage an ongoing client caseload or provide continuing care management after activation. Success in this position requires strong execution, accurate documentation, clear communication, and the ability to work independently while traveling regularly.

Requirements

Key Responsibilities:

• Home Visits: Travel to scheduled locations across Ohio to complete BCI and FBI fingerprint scans, and conduct in-home or community-based activation visits with new clients and caregivers
• Collect Fingerprints:
• Operate mobile fingerprinting equipment accurately and according to established procedures and compliance requirements.
• Ensure fingerprint records and related documentation are complete, accurate, and submitted promptly.
• New client activations: 
• Guide clients and caregivers through required activation steps, forms, acknowledgments, and program documentation.
• Educate clients and caregivers on the onboarding process, program expectations, next steps, and available points of contact.
• Compliance: Maintain compliance with all relevant regulations and standards.
• Role Model: Embody Entyre Care’s mission, values, and culture, acting as a role model for the team

What You Will Bring:

• Bachelor's degree required, must be LSW or LISW, human services, social work, or a related field.
• At least one year of experience working with elders or adults with disabilities.
• Valid driver's license and automobile insurance, as regular travel within Ohio is required.
• Strong organizational and time-management skills.
• Clear, professional, and compassionate communication skills.
• Basic proficiency with laptops, tablets, mobile applications, web-based systems, email, and Microsoft Office.
• Experience working in Structure Family Caregiving (SFC) or a similar setting preferred.
• Certifications in gerontology or case management preferred.
