Jewish Family Service of San Diego (JFS) is seeking a compassionate, mission-driven Care Manager to join the Supporting Our Survivors (SOS) San Diego team. This full-time Care Manager role supports Holocaust survivors through person-centered, trauma-informed care management, including client intake and assessments, care coordination, crisis intervention, resource navigation, service planning, and ongoing short- and long-term support. The position also requires accurate documentation and data entry in care management databases to support grant reporting, program outcomes, and quality services.

Using an outcome-oriented, person-centered, trauma-informed approach, the Care Manager meets with clients in their homes and communities to assess needs, connect them with local resources, coordinate appropriate services, and provide ongoing support that helps each client attain and maintain the highest possible level of independence, stability, and self-sufficiency.

The SOS team provides culturally and linguistically appropriate support through integrated services for vulnerable aging Holocaust survivors, including health and mental health navigation, direct financial assistance, food access, transportation coordination, home care support, and connection to community-based senior services.

In this nonprofit human services role, you will use professional social work, care management, and resource coordination skills to build trust with clients, help ensure benefits and supports are in place, and make a meaningful difference for older adults across San Diego. The Care Manager plays an integral role on the SOS team by supporting program participants, documenting services and outcomes, and helping share impact with grantors and agency leadership. This position reports directly to the SOS Program Manager and may also work with the Director of Holocaust Survivor Programs.

Responsibilities:

• Projects currently include: Claims Conference - Home Care, California Department of Social Services - Holocaust Survivor Assistance Program, Kavod SHEF (Survivors of the Holocaust Emergency Fund) and community supported holiday deliveries.
• Assess each client utilizing social work skills and complete tracking tools in Database
• Determine, best fit for client within program eligibility requirements.
• Use assessment tools and obtain back-up documents per Grant requirements.
• Claims Conference appropriate, obtain all information and releases needed.
• If homecare is needed through grants, complete the necessary forms. Set up and monitor homecare after obtaining approval from program manager.
• If financial assistance is needed through grants, discuss with Program Manager to assure that the survivor is in the lowest income bracket or has an unmet need.
• Document and monitor all submissions for financial assistance.
• Attend quarterly Advisory Committee meetings and share limited details as requested.
• Provide on-going Care Management including crisis intervention, advocacy and communication with family and home care vendors.
• Attend program related meetings including agency’s case consultation meetings, grant progress meetings, and staff meetings.
• Plan and coordinate social events with the Program Manager within budget and grant guidelines.
• Obtain notes of thanks/appreciation from participants and write up short paragraph on their background and the assistance JFS has provided for sharing with funders.

Documentation

• Enter all information in databases and other tracking sheets as needed.
• Participate in community Holocaust events.
• Complete requests for Kavod SHEF and other grants (such as Blue Card applications)
• Work on additional assignments, as appropriate.
• Maintain confidentiality and quality in all services with and for clients.
• Provide comprehensive assessments of each client, identifying areas of need and partnering with clients to develop individual care plans to set and achieve realistic and time specific personal goals.

Client Records

• All client records must be comprised of all required forms 100% of the time unless documentation indicates special circumstances where it was impossible to complete the form.

Confidentiality

• Client must sign a "Release of Information" prior to any information relative to the client being discussed with anyone outside the employment of the agency 100% of the time (APS and life endangerment--Tarasoff --excluded).
• Client must sign a Media Release before any photo or video can be shared outside the agency.

Quality Assurance

• Regular review of case files (physical and Electronic) to ensure all documents are there and ready for audit at any time.
• Send client thank you notes or emails to be saved to client files with background details to be available for reporting purposes.
• Client satisfaction surveys, pre and post surveys as needed per project.

Skills/Experience/Abilities That Are a Must-Have:

• Bi-lingual – Russian preferred.
• Strong communication skills and attention to detail
• BSW or Bachelor’s degree in Gerontology or human services field, MSW or Masters preferred.
• Minimum of two years of experience in care management/resource coordination role.
• Trauma-informed regarding the issues of Holocaust survivors.
• Experience working with the elderly, particularly those with PTSD.
• Significant familiarity with Jewish history, traditions, and culture.
• Valid driver's license.
• Dependable accessible vehicle that can be used to transport clients
• Current auto insurance and clean DMV record.
• Ability to work with multiple software systems, including complex care management database.
• Comfort in providing services to clients in person and in community, as needed.
• Knowledge of Jewish culture and the local Jewish community preferred.
• Desire to work with a diverse and integrated team.
• Ability to work flexible hours, including some weekend and evenings hours.
• Motivated as a self-starter
• Knowledge of Microsoft suite of computer programs

Skills/Abilities We’d Like You to Have:

• Fluency in Russian Ukrainian or European languages
• Knowledge of local state and federal benefits for low-income senior citizens
• Time management and organizational skills

• Writing skills
• Creative design for use of Canva or similar platforms

Physical Requirements:

• Must be able to drive and transport passengers
• Sit or stand for long periods of time
• Lift packages of 15-20 lbs

Pay Range: $27 to $29/hour
