In this role, you'll combine your clinical expertise with care coordination, coaching, education, and resource management to support high-risk and medically complex patients throughout their healthcare journey.

As a Care Manager RN, you will:
• Manage a panel of high-risk, high-cost, and chronic condition patients
• Assess patient needs and develop individualized, patient-centered care plans
• Collaborate with primary care physicians and multidisciplinary teams to coordinate care
• Coach and educate patients on disease management, preventive care, medications, and self-management strategies
• Connect patients with community resources and support services
• Promote engagement in healthy behaviors and self-care activities
• Facilitate smooth transitions across the continuum of care, including acute care, rehabilitation, skilled nursing, home health, and outpatient settings
• Monitor patient progress and adjust care plans to support optimal outcomes
• Utilize evidence-based practices to improve quality, efficiency, and patient satisfaction
• Advocate for patients, families, and caregivers throughout their healthcare journey

• Associate Degree in Nursing (ASN)
• Bachelor of Science in Nursing (BSN)
• ASN with 10 years of acute care nursing experience; OR
• BSN with 5 years of acute care nursing experience
• Active Registered Nurse license in Maryland or qualifying Nurse Licensure Compact (NLC) multi-state license
• Current BLS/CPR certification
• Ability to obtain active Pennsylvania and West Virginia RN licenses within 6 months of employment
• Care/Case Management Certification required within 5 years of employment
• Care management, case management, managed care, patient-centered medical home, or integrated care management experience
• Experience working with chronic and complex patient populations
