This position is all about relationships. You'll be the face of the agency to care managers, case managers, insurance plan reps, and other referral sources, building the kind of trust that turns a single introduction into a long-term partnership. Key Responsibilities

Relationship Building

• Develop and maintain strong, ongoing partnerships with care managers, case managers, and insurance plan representatives
• Become a trusted point of contact for referral sources across the network
• Connect authentically with a wide range of personalities and professional backgrounds

Outreach & Network Expansion

• Research and identify potential new referral sources
• Conduct proactive outreach to bring new partners into the fold
• Develop creative strategies to strengthen existing relationships and grow the referral network

Referral Generation

• Drive consistent patient referrals through relationship-driven engagement
• Nurture connections over time rather than treating any interaction as a one-time touch

Qualifications

• Strong interpersonal and networking skills — you genuinely enjoy building relationships
• Self-motivated and proactive, not waiting to be told what to do next
• Creative and dynamic in your approach to outreach and engagement
• Quick thinker with solid emotional intelligence
• Excellent communication skills, both verbal and written
