Job Description

Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who see it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Social Care Network Care Manager to join our Health Home team in Suffolk County.

Social Care Network Care Manager (SCN Care Manager) – Temporary

Role Details

The SCN Care Manager supports the implementation of a patient’s integrated health care plan and works toward resolving patient needs and barriers. This role conducts screening and navigation services for patients eligible for the Social Care Network, with documentation completed in Unite Us and eCW. The focus is on providing individualized assistance to help patients navigate the health care system and community barriers, facilitating consistent and timely care across the continuum.

Job Responsibilities / Competencies

  • Perform Health‑Related Social Needs (HRSN) assessments and document findings in Unite Us and eCW
  • Provide general navigation services and communicate program objectives to patients
  • Assist with scheduling appointments related to social drivers of health, including reminder and follow‑up calls
  • Contact patients to support continuity of care and escalate cases requiring enhanced care management
  • Compile and distribute educational materials based on patient needs
  • Identify individual and community resources that support healthy lifestyles (e.g., transportation, exercise programs, community services)
  • Maintain timely and accurate documentation of patient interactions
  • Engage approximately 25 new patients per week
  • Perform population management activities as assigned

  • Ability to work independently and collaboratively
  • Strong time‑management and problem‑solving skills
  • Patience and capability to navigate complex systems of care
  • Proficient computer skills, with the ability to document concurrently in Unite Us and eCW

Minimum Qualifications

  • Education: High School Diploma (required)
  • Language: English (required); Spanish (preferred)
  • Work Location: Onsite

Status: Temporary

Pay Range: $20/hour

About Sun River Health

In the early 1970s, four African American women in Peekskill, New York came together to address the lack of affordable, quality health care in their community. With money from a small federal grant, the first Sun River Health site was opened in 1975.

That first health center has since grown to become an FQHC (federally qualified health center) system with over 40 locations, serving more than 250,000 patients throughout the Hudson Valley, New York City, and Long Island. Our exceptional primary care practitioners, specialists, and support staff have made us a destination for affordable, high-quality care.

In December 2018, Brightpoint Health, an FQHC network offering integrated medical, behavioral, and social support services in New York City, merged with Hudson River Health Care. Together with our subsidiary organizations, we became Sun River Health in December 2020.
