Responsibilities:
Presbyterian Healthcare Services is seeking a dedicated Behavioral Health Therapist or licensed Social Worker to serve as a key member of the interdisciplinary care team as Care Manager, providing comprehensive therapeutic services to individuals experiencing mental health and behavioral health challenges. In this impactful role, you will guide patients through assessment, treatment, crisis intervention, discharge planning, and recovery while partnering closely with physicians, nurses, case managers, and other healthcare professionals.
As a Care Manager, you will play a vital role in helping patients achieve their treatment goals through individual counseling, group therapy, case management, and care coordination. You will advocate for patients throughout their hospitalization, ensuring access to appropriate services, facilitating smooth transitions of care, and supporting positive clinical outcomes. This position combines direct patient care, clinical expertise, utilization management, and multidisciplinary collaboration in a dynamic behavioral health setting.
