The Care Manager III (CMIII) provides care management services including completion of intake services and care coordination for clients in San Francisco.

The CMIII is a senior, autonomous role that supports complex cases across multiple programs serving older adults and adults with disabilities. This position ensures quality and regulatory compliance, provides mentorship to staff, and serves as a resource to promote coordinated, high-standard care across teams.

ESSENTIAL JOB FUNCIONS:

  • Completes assessment and care plans as indicated by supervisor
  • Ensures program and staff compliance with all regulatory requirements.
  • Ensures quality care and services to all clients.
  • Engages in crisis intervention to support client needs as needed
  • Responsible for mentoring new staff during on boarding process
  • Maintains Quality Assurance mechanisms to measure and maintain high standards of care and programming, including regular chart reviews and peer review.
  • Complies with all mandated reporting to support local and state requirements
  • Develops and maintains collaborative working relationships and cooperative arrangements with team, supervisor, community individuals, groups and organizations
  • Takes advantage of opportunities to educate the public and the community regarding program.
  • Responsible for ensuring that authorized and purchased goods/services are used as a last resort and that all community resources and referrals are made to appropriate waiver programs.
  • Actively participates in and encourages actions that promote good public relations with clients, their families and the community.
  • Responsible for expanding personal knowledge, skill and attitudes specific to care management, family systems, geriatrics and gerontology, addiction and other relevant management or health care issues.
  • Provides caseload coverage when Care Management staff are out of office or unavailable
  • Accepts and performs other related duties and responsibilities as required.

EDUCATION: MSW or Master's in related clinical field (ex: MFT) , current license (LCSW, LMFT, or RN) preferred

BACKGROUND AND EXPERIENCE:

  • Five years' experience working directly with disabled/older adults or related social/ medical experience required with at least one year demonstrated leadership skills.
  • Demonstrated ability to supervise intensive care management staff who work with adults with disabilities at risk of hospitalization, which present with complex medical conditions, mental health diagnoses, substance abuse, as well as physical rehabilitation needs.
  • Demonstrates understanding of behavioral health services and behavioral health interventions.
  • Proven previous training of staff as it relates to the populations served.
  • Demonstrates understanding of the health and psychosocial needs of this population.
  • Displays knowledge of community resources available for this population.
  • Demonstrates ability and willingness to take on program development projects.
  • Proven ability and willingness to provide leadership amongst peers and mentor junior staff.

COMPENSATION:

  • Range: $ 95,170.00 - $ 113,742.00annual
